BackgroundRespiratory diseases (RD) can challenge healthcare systems around the globe. Natural health products (NHPs) are popular complementary and alternative medicine options for health issues concerning non-fatal RD. Little is known about the characteristics of the users of RD-NHPs and about their NHP consumption behavior during the Covid-19 pandemic in Germany.MethodsA representative online survey was conducted in Germany in 2022. 1707 participants were classified based on having used NHPs for RD within the previous 12 months, having used NHPs but not for RD within the previous 12 months and not having used NHPs. Data were analyzed using descriptive and inferential statistical methods as well as a multinomial logistic regression model.ResultsUsers of RD-NHPs within the previous 12 months were more likely to be employed and to consult pharmacists more often for non-fatal health issues than individuals who did not take RD-NHPs. RD-NHP users were more likely to suffer from a Covid-19 infection and to have children living in the same household than other NHP users. Compared to non-NHP users, RD-NHP users were more likely to be female, highly educated and have stronger openness-to-change value orientations. Vaccination-related behavior was no indicator of RD-NHP usage. Most RD-NHP users took NHPs in self-medication. Few reported informing their practitioner about their self-medication. Drugstores were the most visited supply source for NHPs during the pandemic, followed by pharmacies. Common information sources regarding NHPs were the products themselves and pharmacists.ConclusionThis study emphasized the important role of NHPs as a popular prevention and treatment option for RD. RD-NHPs were more likely used by individuals who were employed, who suffered from a RD and who consult pharmacists for non-fatal health issues. The importance of product information and pharmacies as information sources should be considered to make communication strategies about safe self-medication options with RD-NHPs more effective, which could help to reduce the burden of health facilities regarding non-fatal RD. To improve and develop future pandemic-control strategies, health professionals and policy makers should consider NHP usage behavior and provide critical information about chances and risks of self-medicated NHP consumption.
Dieses smart VHB-Modul thematisiert die statistische Auswertung ökologischer Datensätze. Dabei liegt der Fokus auf der Anwendung von Verfahren. Von der Grundidee der statistischen Auswertung über die Skalierung von Daten und die Anforderung an Datensätze geht es zu den statistischen Ansätzen. Hier werden die Darstellung und Kennzeichnung von Verteilungen, Ansätze zu Mittelwertvergleichen, Anwendungsfälle der Varianzanalyse und der Regressionsanalyse ebenso vorgestellt wie die besonderen Anforderungen an mehrfaktorielle bzw. multiple Analysen. Dazu werden ein kurzes Skriptum und die nötigen Datensätze angeboten. Zur Umsetzung der Analysen in R werden drei weitere smart VHB-Module angeboten. Die smart VHB-E-Learning-Module sind für alle Mitglieder der Bayerischen Hochschulen frei zugänglich und können als edu-sharing-Komponente auch in moodle-Kursräume eingebunden werden.
Dieses smart VHB-Modul thematisiert die statistische Auswertung ökologischer Datensätze mit dem frei verfügbaren Statistikprogramm R. Dabei liegt der Fokus auf der Anwendung von Verfahren. Das Statistikprogramm R wird vorgestellt. Dazu werden die Benutzeroberflächen R-Commander (gut geeignet für Standardanwendungen) und RStudio (geeignet für komplexere Analysen) vorgestellt. Erste Schritte der Anwendung werden behandelt. Für die intensivere Beschäftigung mit der Umsetzung von Analysen im R-Commander gibt es ein zweites Modul. Für die Arbeiten in RStudio gibt es ein drittes Modul. Zur Auswahl des geeigneten statistischen Verfahrens wird ein weiteres smart VHB-Modul angeboten. Die smart VHB-E-Learning-Module sind für alle Mitglieder der Bayerischen Hochschulen frei zugänglich und können als edu-sharing-Komponente auch in moodle-Kursräume eingebunden werden.

Assigning inpatients to hospital beds impacts patient satisfaction and the workload of nurses and doctors. The assignment is subject to unknown inpatient arrivals, in particular for emergency patients. Hospitals, therefore, need to deal with uncertainty on actual bed requirements and potential shortage situations as bed capacities are limited. This paper develops a model and solution approach for solving the patient bed-assignment problem that is based on a machine learning (ML) approach to forecasting emergency patients. First, it contributes by improving the anticipation of emergency patients using ML approaches, incorporating weather data, time and dates, important local and regional events, as well as current and historical occupancy levels. Drawing on real-life data from a large case hospital, we were able to improve forecasting accuracy for emergency inpatient arrivals. We achieved up to 17% better root mean square error (RMSE) when using ML methods compared to a baseline approach relying on averages for historical arrival rates. We further show that the ML methods outperform time series forecasts. Second, we develop a new hyper-heuristic for solving real-life problem instances based on the pilot method and a specialized greedy look-ahead (GLA) heuristic. When applying the hyper-heuristic in test sets we were able to increase the objective function by up to 5.3% in comparison to the benchmark approach in [40]. A benchmark with a Genetic Algorithm shows also the superiority of the hyper-heuristic. Third, the combination of ML for emergency patient admission forecasting with advanced optimization through the hyper-heuristic allowed us to obtain an improvement of up to 3.3% on a real-life problem.
